In the event of an incident/injury, the club staff must complete and submit the Injury Report Form to Gymnastics Ontario with 2 days of the incident
Filing a Sport Accident Insurance Claim
This coverage is secondary to any other health care plan(s). Expenses eligible under any other health care plan(s) must be submitted to that plan(s). To make a claim, you must exhaust provincial health care and any extended health benefits before you submit.
Gymnastics Ontario registered members looking to be reimbursed for medical expenses incurred during a Gymnastics Ontario sanctioned activity and not covered by their primary insurance provider must take the following actions
- The club representative must complete the Gymnastics Ontario Injury Report and submit to Gymnastics Ontario with 2 days of the injury.
- The club representative must advise Gymnastics Ontario with 30 days of the injury that the Family will be submitting a Sport Accident Claim Form
- The injured individual or guardian should have the Sport Accident Claim Form completed and submitted to Gallagher within 90 days of the injury. Instructions to submit this form are located on page 3 of the Sport Accident Claim Form.
General Sport Liability and Director’s & Officers’ and Employment Practices Liability
Gymnastics Ontario member clubs who have been served with a lawsuit must contact Gymnastics Ontario’s insurance provider Arthur J. Gallagher Canada Limited immediately to begin the claim process.
